To create the Forward to Friends Form, click the "Select" button on the Create Form page and work your way through the simply create the form through the different steps of the Form Editor (Fig 1).

The Forward to Friend Form is used in a slightly different way. You create the form using the same process as the other forms, but when you wish to use it, you need to include it in your email campaigns with a link you add to the template. The link is created so you can record which contact forwarded the email and the time it was forwarded. 

You can select the name of the form from under "Insert Form" in the DND Builder. Click on the form you want and insert its name into your email campaign. At the time of sending, it will be converted into the link and sent on to your contacts.
