An Embedded Signup Form allows visitors to your site to subscribe to your contact list. You can gather information on your contacts from any custom fields that you have created and included in your form. 

To create this form, click the "Select" button on the Create Form page and work your way through the simply create the form through the different steps of the Form Editor (Fig 1).

After you're done creating your Embedded Signup Form, you be redirected to Add the Form to Your Website (Fig 2).

Follow the instructions at the top of the page to copy the code so that you may place the code on your website. 

Click the "OK" button to be redirected to the View Website Forms page (Fig 3).
