To create this form, click the "Select" button on the Create Form page and work your way through the simply create the form through the different steps of the Form Editor (Fig 1).

You can include this form in your email campaigns by inserting the form's name into your template. Just like the Forward to Friend form, the form's name will be converted into a unique link so that your contacts can log in and manage any details that you included when you created it (contact lists they are subscribed to, what details are stored for their custom fields, email address, etc.).
