To create this form, click the "Select" button on the Create Form page and work your way through the simply create the form through the different steps of the Form Editor (Fig 1)

The Embedded Unsubscribe Form is created and used in much the same way as the Embedded Signup Form except that you cannot add custom fields to it. Once it is created and placed on your website, you can direct your contacts or site visitors here so they can unsubscribe from your contact list.
